To start a new page, you can start from a link to the title of the new page. In order to do so you can edit an existing page first and fill in a link to the new page you want to create by using the [W] function.
Internal links to non-existing pages are typically created in preparation of creating the page, and/or to encourage other people to do so.
Another way is to search for the expression or title first. If you do not find an existing match click "write a new article" in order to create a new article with the desired title.
